Advance Care Planning Workshop

"Advance care planning" is the conversations and steps involved in making a "living will" or "advance health care directive." It is important to have an advance care plan in case you cannot speak for yourself. It can help provide comfort because you and your loved ones know your wishes for care, no matter what happens.

This 2-session workshop helps you decide what's right for you and those close to you. It is for people living with or caring for a person with a serious illness. It is also open to people who are healthy.  

Come learn about the questions you should be asking yourself, your doctors, and your loved ones. You may even leave with an advance care plan completed.
